Qualifications for Admission to Broadview:

• Radical reliance on Christian Science for healing.
• Daily treatment from a Journal-listed Christian Science Practitioner.
• Freedom from use of medication and medical equipment or devices.

Documents Required for Admission:

• Social Security card 
• Medicare card, if applicable
• State Drivers’ License OR State Identification card OR U.S. Passport (current or issued within the last 5 years) OR Two persons unrelated to the patient to sign a notary attesting to the patient’s identity.
• A copy of patient’s Advance Health Care Directive, if the patient has one.

Required private health insurance information for each past or current policy:

• Health Insurance card or billing statement
• Name of insurance company
• Address & phone number/s
• Policy numbers
• Group number
• Beginning and end dates of coverage
• Identify policies as either: HMO* (Health Maintenance Organization), PPO (Private Pay Organization) or Supplemental Insurance policy

* If the patient has an HMO, written authorization must be obtained from the HMO before coming to Broadview.  Otherwise, expenses at Broadview will not be covered and will be the responsibility of the patient and family.
